What is the PICSI?

PICSI (Physiological IntraCytoplasmic Sperm Injection) is an advanced version of the ICSI (intracytoplasmic sperm injection) technique used during IVF. It involves selecting the most mature and genetically healthy sperm using a special hyaluronic acid (HA)-based dish, which mimics the natural binding environment found in the female body. Only sperm that bind naturally to hyaluronic acid are selected for injection into the egg.

These sperm are believed to have lower DNA fragmentation, better maturity, and improved fertilization potential, leading to healthier embryos and higher success rates.

At Zemya IVF & Fertility Clinics, we offer PICSI in Delhi as part of our advanced sperm selection protocols for couples undergoing IVF or ICSI, especially those with male factor infertility or repeated treatment failures.

Why is PICSI Done?

While traditional ICSI uses sperm selected under a microscope based on shape and movement, it doesn’t reveal the genetic quality or maturity of the sperm. PICSI helps overcome this limitation by providing a more physiological, natural selection method. PICSI is recommended for:

  • Couples with repeated IVF or ICSI failure
  • Cases of poor embryo development or fertilization issues
  • Male infertility with abnormal semen analysis
  • High levels of DNA fragmentation in sperm
  • Recurrent miscarriage likely linked to sperm quality
  • Couples opting for PGT-A/PGT-M who want to ensure the best sperm is used for fertilization

Benefits of PICSI in IVF

  • Helps select mature, functional sperm that are less likely to carry DNA damage
  • May improve fertilization rates and embryo quality
  • Reduces the risk of genetic abnormalities associated with poor sperm
  • May lower the chances of miscarriage
  • Supports better blastocyst development in extended embryo culture
  • Complements PGT cycles by providing genetically healthier starting material

While PICSI does not guarantee success, it offers an important tool for couples where sperm quality is a concern.

PICSI Procedure Step-by-Step Process

PICSI is integrated into a standard IVF/ICSI cycle, but involves a specialized sperm selection method before fertilization.

Step 01

Semen collection and preparation

  • A semen sample is collected from the male partner after 2–5 days of abstinence.
  • The sample is processed in the andrology lab to separate motile sperm.

Step 02

PICSI dish preparation

  • A specially designed PICSI dish contains dots of hyaluronic acid gel, which mimic the natural binding environment of a mature egg.

Step 03

Sperm selection

  • The processed sperm sample is introduced into the PICSI dish.
  • Only mature sperm bind to the HA dots, as they have specific receptors that indicate better genetic integrity.
  • Sperm that fail to bind are considered less mature or potentially fragmented.

Step 04

Injection into the egg

  • Embryologists select the HA-bound sperm for microinjection into the egg, following the ICSI procedure.
  • This step ensures that only the most suitable sperm is used for fertilization.

Step 05

Embryo culture and transfer

  • The resulting embryos are cultured, monitored, and graded for quality.
  • Embryo transfer is scheduled based on embryo development, usually on day 3 or day 5.
  • Remaining embryos can be frozen for future use.

PICSI vs ICSI – What’s the difference?

Feature ICSI PICSI
Sperm selection Based on appearance and motility under a microscope Based on natural binding to hyaluronic acid (mimics egg environment)
Genetic quality assessment Limited Higher chance of selecting sperm with intact DNA
Ideal for Most IVF/ICSI cases Recurrent failure, male factor infertility, high DFI
Goal Fertilization Improved fertilization and embryo quality

PICSI is a refined version of ICSI and may offer improved outcomes in select patients.

Do’s and Don’ts before PICSI Procedure

Do’s:
  • Follow all semen collection guidelines (2–5 days of abstinence)
  • Maintain a healthy diet and lifestyle in the weeks leading up to treatment
  • Avoid exposure to heat, radiation, and toxins that may harm sperm
  • Discuss DNA fragmentation test results with your doctor if previously done
Don’ts:
  • Avoid smoking, alcohol, and recreational drugs
  • Don’t delay testing if there’s a history of male factor infertility
  • Don’t stop medications or supplements without consulting your fertility specialist

Frequently Asked Questions

PICSI offers an additional layer of sperm selection, making it better in cases of poor sperm DNA quality, recurrent IVF failure, or poor embryo development. It may not be necessary for all IVF cycles.

No. While it can improve fertilization and embryo quality, pregnancy still depends on egg quality, embryo implantation, and overall reproductive health.

Yes. PICSI is a non-invasive sperm selection method and does not harm the sperm or egg. It uses natural biological processes to choose the best sperm.

Yes, PICSI is an add-on procedure in IVF. At Zemya IVF & Fertility Clinics, we provide affordable PICSI treatment in Delhi, with full transparency about when it's medically indicated.

Not always, but it can be helpful. If high fragmentation is found, PICSI may be strongly recommended to improve embryo quality.

Yes. PICSI is often used in PGT cycles to ensure only genetically healthy sperm is used to fertilize the egg before embryo testing.
